The report says there were gaps in coordination among the four top doctors including Khan. Government officials had earlier held Khan responsible for the oxygen shortage.<\/p>\n<p>The <a href=\"https:\/\/apnlive.com\/topic\/uttar-pradesh\">Uttar Pradesh <\/a>Health Minister Siddharth Nath Singh has earlier admitted that the loss of lives due to oxygen shortage was nothing less than a \u2018heinous crime\u2019.<\/p>\n<p>Meanwhile, five more encephalitis- related child deaths have been reported from the hospital on Wednesday rising the toll to seventy-one since August 7.
